I did not know just what or hoe much care was available before my wife because terminally ill, and the high level of support enabled me to keep her at home night through to her death.

Suggested improvements
I know it's difficult to predict, but the earlier that this provision can be advised to the carers/relative the better our planning can be.

Thank you for your feedback. We work closely with other services within NHFT and encourage them to advise which services are available for patients who are nearing the end of their life. We will ensure we continue to update community nursing teams to encourage them to advise when Hospice at Home team would be appropriate

The support I had at the end when the team became involved was such a great help and helped me cope with my husbands last few days. I do not know how I would have coped without it. They were so kind and caring and treated him with such dignity.

My husband was treated with care and compassion at al times. Without the carer's I could not have cared all who came were wonderful and kind and thoughtful to the end!

The nurses were very caring. Thanks to their involvement we were able to care for my husband at home in his final days.

Suggested improvements
The care of hsopice at home nurses was excellent, but they were hampered by the poor attention to detail of the GPs who seemed unable to complete the drugs prescription correctly. It took 3 attempts and hours of the nurses time to rectify GPs mistakes.

Thank you for your feedback. We work closely with our GP colleagues and try and provide clear instructions on what medications are required. It would be good to provide this feedback to your GP practice so they can also learn from this. As this is anonymised feedback we are unable to ascertain which GP practice this comment relates to.

The service provided was excellent and my family and myself will always remember it friendliness of all the people { } on last few weeks of mylots { } life. The help was very much appreciated.

I found the staff of the palliative care team very helpful sympathetic and informative.

As this is the first time I have had to deal with such a situation as this. I do not think the team could have done any better.
